Our health care professionals want your visit to be as comfortable and productive as possible

• When you make an appointment, identifying the specific reason for your
   visit will help our staff schedule the proper amount of time. If you are new
   to the Clinic, please arrive 15 minutes early in order to complete the
   necessary initial paperwork.

• Be sure to bring all of the medications that you are currently taking,
  or a list of the medications that includes the prescription name, dosage
  and number per day.

• AND OF MOST IMPORTANCE:
  If you have a prescription that will run out before your next scheduled
  appointment, please call your pharmacy 72 hours prior to need. The
  pharmacy will FAX a written order to our office, which will be refilled as
  soon as possible.
