HOME
HEALTH & HOSPICE
At some time, you or a member of your family may experience an
illness or injury that requires professional medical attention,
but not necessarily care in a hospital setting. Home Health offers
coordinated health services to meet the needs of homebound patients,
and to provide support for family caregivers.
Referral for
Home Health services can be made by a primary physician, family
member, friend or patient. Upon referral, Home Health arranges
for an in-home assessment visit by a Registered Nurse. This assessment
will determine if the patient's medical, nursing, social and related
healthcare needs can be adequately met within the home environment.
Once eligibility is confirmed, Home Health service coordination
is provided by an RN in consultation with the patient's physician.
Services may include:
Skilled
Nursing Services • Home Health Aide Services
Medical Social Worker • Speech Therapy
Physical Therapy • Occupational Therapy
Who
is Served by Home Health?
Services are available to adolescents and adults who
reside within Klickitat County and are homebound with an acute
or changing medical condition, (as opposed to stable), that requires
a skilled healthcare service. Services must be given under a physician's
written plan of treatment, however your physician need not be
local, and may be associated with any hospital in any state.
Reimbursement
for Home Health services is available through Medicare, Medicaid
and most insurance companies. Preauthorization of services may
be required by some private insurance plans. The patient is responsible
for the balance of services not covered by their health plan.
Our staff of health care professionals continually assesses, plans,
implements and evaluates our policies, procedures and services
to ensure the quality care that patients deserve. Home Health
of Klickitat Valley Health is currently licensed in accordance
with applicable laws of the State of Washington.
HOSPICE
SERVICES
Home Health also offers Hospice Services for patients and their
families seeking terminal care and support. Learn more about how

parking lot on the KV Health campus at 310 S Roosevelt.
The office is open Monday thru Friday 7:00 am – 5:00 pm
